Ways to Contribute
to the
Pennsylvania Health Law Project
There are two ways you
can make a contribution to the Pennsylvania Health Law Project:
You can mail your
tax-deductible donation - check or money order - directly to us at
our Philadelphia office:

If you
would like a letter acknowledging your donation for tax purposes,
we will be happy to furnish one upon your request.
You may also make donations
to the Pennsylvania Health Law Project
through the United Way Donor's Choice Program. If your employer
participates in this program, please ask for an application form
and enter the Pennsylvania Health Law Project
number, 10277. The amount you choose to designate
with be deducted from your paycheck. Some employers provide matching
gifts.
If you are outside of
the Southeast Pennsylvania Region, please check with your local
United Way on how to donate to the Pennsylvania Health Law Project
through Donor Choice. To locate your local chapter of the United
Way, click here.
The form below has
been provided for illustrative purposes only! Please use
it only as a guide to help you fill out your form. You will substitute
your own name, employer, social security number and total annual
gift amount for the examples given.
