Share Your Story

If you are having problems receiving treatment, or have bills you can not afford, because you are underinsured or uninsured, the Hospital Accountability Project (HAP) wants to hear your story!!! Please help HAP improve hospital policies by sharing your story. You can share your story three ways:

(1) BY MAIL or FAX

Complete the Share Your Story form, and fax it back to HAP at 215-625-3879 or mail it to Leonardo Cuello, PHLP, 437 Chestnut Street, Suite 900, Philadelphia, PA 19106.

(2) BY EMAIL

When did you go to the hospital?

Did you receive treatment at the hospital?

Have you received bills from your hospital visit?

How much does the hospital say that you owe?

When you received treatment from the hospital, did you think that you had health insurance that should have covered the bills?

Has someone at the hospital talked to you about applying for public health insurance, like Medical Assistance (Medicaid), to cover the bills?

Did someone at the hospital tell you that you might be able to get free care or discounted care from the hospital?

Since you began receiving hospital bills, have you contacted someone at the hospital to tell them that you need help paying the bills?
